The name of field #15 is image (same as the label, only lowercase).

strange ... maybe a conflict name ....
can you try to rename it like content_image (becarfull you will lost content for each item)

FLEXIcontent is Free but involves a very big effort on our part.
Like the our support? (for a bug-free FC, despite being huge extension) Like the features? Like the ongoing development and future commitment to FLEXIcontent?

Renaming the field in the production site is not an option, I'm afraid.
Thousands of items use the field and we cannot afford to lose it.

I will change the field name in the development site just to move the investigation further, but In my view this issue is caused by a bug in Flexisystem (or elsewhere in FC).

Changing the field name does not make any difference.

I have renamed main_image to cover_image, but Flexisystem sill looks for the field named image

Quick update.
There is something else at play here.
It would appear that the image files disappeared from their usual path (or that the correct path got override in FC).

UPDATE 2

The 3 versions of each image file are where expected (images/stories/flexicontent).
However, even those items where the image path is stored in the database (table qgh6_flexicontent_fields_item_relations; column value) keep showing the notice attached below-
