Since sh404SEF has gone commercial I do not have access to it to be able to test.
You should ask in their support forum.

Hi,

Support Plugins for SH404 can be in the sef_ext folder of the component or in the sef_ext folder of the SH404 component.

I think that the component folder is checked first by SH404.

There was a little bug in the SH404 plugin. I think it's fixed (but i have not do lots of tests).

You have to checkout the latest SVN version or read the issue 51 in the bug tracker.
And purge the url in SH404.

It works for me. I hope for you too.
