I fixed it!

It's becouse of some weird strato server....
I had to manually set

 $PHPTHUMB_CONFIG['document_root']

Hello

we have a file for setting such settings  when needed
phpThumb.config_OVERRIDE.php
you must put it there
and not in phpThumb.config.php  !!

and yes sometimes you need to set

$PHPTHUMB_CONFIG['document_root']
Indeed in some servers the automatic setting of the above is not set correct
please tell us details about the server that you are using

Server type and OS version
PHP type and its version number

phpversion  7.0.31 $_SERVER[SERVER_SOFTWARE] = Apache/2.4.34 (Unix)
I used this php code to figure out what document_root really is
Code:
<?php echo $_SERVER['DOCUMENT_ROOT'].$_SERVER['PHP_SELF']; ?>
Then set the correct path into 
Code:
$PHPTHUMB_CONFIG['document_root']
in phpThumb.config_OVERRIDE.php
