guests can create items if you have given them 'create' privilege in at least in category
or
if you have created a menu item with "submit override" and a guest have clicked it

Well, I had two questions in this topic. Micker replied to the second question which was about limiting items in a category so only the author can see their own items. This is essentially an ACL question.
