Top level categories list no back option and incorrect items

I have created a menu item that lists all of the categories of content.

This displays them all as a list which is great. The problem I have is that when I select a category and display the list there is no option to go back to tphe previous screen. I have to reselect the menu item to get the list of categories back.

My categories are configured so that some are public and some protected. When not logged in only the public ones are displayed and the item count shows correctly. When I login and select the menu all of the categories are shown but the item count is set to 0 on all of them.

I have attached some screen shots to demonstrate the issue I am having.
